Since 2014 the American Heart Association has celebrated the achievements of prehospital providers and their destination hospitals specific to STEMI patient care through its Mission: Lifeline EMS Recognition program. The application period is open now through April 2, 2019 and we hope you’ll consider this opportunity to receive recognition for your contributions as a vital member of our STEMI team.

Application: There are two ways to access your application:
1. Previous Mission: Lifeline EMS applicants:
An email was sent 9/10/2018 with a 2019 recognition application link, unique to the applicant agency. This email was sent from AHASurveys@surveys.heart.org and was sent to the contact listed on last year’s application. Please be sure to check your junk and/or spam email folders just in case the email was filtered to one of them. If you did not receive the email, please email firstname.lastname@example.org and she can provide you with a copy of the link so you can get started!
2. Agencies who have not previously applied for Mission: Lifeline EMS recognition:
Please complete the 2019 Mission: Lifeline EMS Application Request form. Upon completion of this form an application link will be sent via email within 2-business days. This link will provide access to an open-ended application where data can be submitted on a regular basis -quarterly, bi-annual or yearly. All information and data entered is automatically saved within the application when the browser is closed, and applicants can return to the application as needed to enter more data, make corrections, etc.
